Enable people with health risks to become active via referrals to certified coaches.

After being referred, participants engage in more physical activity through personalized plans tailored to their daily lives (e.g., walking, cycling, gardening, swimming).

Participants move more after referral and coaching; healthy habits are incorporated into daily routines.

~ 85% fully reach their personal activity plan goals; ~12% partially reach them.

They achived healthcare integration: multiple healthcare professions can refer patients.

Subsidized and broad eligibility allows wider access across risk groups.
